Rite Aid just posted the following on their Facebook page:
We apologize for any difficulties you are experiencing with the $3 off a $15 purchase coupon offered for participating in our customer satisfaction survey. The offer is no longer available, but our stores will honor any receipts that have the survey invitation already printed on them. Simply present the receipt with the survey invitation at the time of checkout.
Yay, that will save us time and ink! 
wish i would have seen this yesterday, i had two coupons with surveys, did both surveys, didn’t get the coupons, and threw them away because I thought I might have missed the time period to fill out the surveys…
hi ..i walked in a rite aid today & attendant said you must print coupon.. he was not aware of anything mentioned here….thanks in advance
